Confirmed chartered 12/16/1801.
1801 Proceedings: A dispensation, dated March 28th, A. L. 5801, having been issued to Brothers Reuben Fry, as Master; John Booker, as Senior Warden; and Churchill Gibbs, as Junior Warden; with authority to open a Lodge at Madison Courthouse, by the name of the Madison Lodge; and the said dispensation, with a copy of the proceedings had thereon, having been received, with a petition for a charter:
Resolved, That a charter do issue for the permanent establishment of said Lodge, appointing John Booker, Master; Leonard
Barnes, Senior Warden; and Churchill Gibbs, Junior Warden; by the name of the Madison Lodge, No. 64.
1812 Proceedings: It being represented that Madison Lodge, No. 64, whose communication with the Grand Lodge has, for some time past, been suspended, owing to the dispersed situation of its members and the dormant state of the Lodge, had during the present communication been duly represented and paid the sum of fifty dollars into the hands of the Grand Treasurer.
On motion,
Resolved, That the said sum be received in full discharge of all arrearages due from said Lodge.
